    The P53 protein is expressed in all normal cells and appears to function in cell cycle regulation. Abnormally higli levels of the protein are found in many different types of cancer. In breast cancer, overexpression of P53 is associated with point mutations within highly conserved regions of the P53 gene. These altered genes encode stable P53 protein that can be detected by standard immunohistochemical techniques.
    In this study, we examined 47 cases of primary breast carcinoma for the presence of P53 protein using immunohistochemistry methods employing monoclonal antibody against the clone, DO-7. Of these specimens, 25.5% had widespread overexpression of P53. A significant positive correlation waz found between P53 overexpression andyounger age (P<0.05). There was a tendency for P53 overexpression in premenopausal women and the higher tumor grades, although! these did not acheive significance. The P53 overexpressoin was not correlated with tumor size, tumor type, nodal status and side of involved breast. The overexpressoin ofP53 may itself be a prognostic factor in human breast cancer.

    Due to the importance of clinically significant macular edema in diabetic patients, this study is aimed to determine if laser photocoagulation is effective in the treatment of clinically significant diabetic macular edema. In addition, the effects of risk factors arc surveyed* This is an existing data study considering patients with clinically significant diabetic macular edema, treated with argon-green laser photocoagulation in Labbafinejad hospital, department of lasertherapy, from 1995 to 1997. in 60 (42.6%) eyes the treatment method was focal, in 22 (15.6%) eyes grid, and in 59 (41.84) modified grid laser photocoagulation was performed. The results are based upon deterioration of visual acuity, occurance of moderate visual loss and improvement or persistence of CSME. We studied 114 eyes from 87 patients. Two years after initial treatment, visual acuity improved in 19.1% of eyes, unchanged in 9.5% and worsened in 71.4% of eyes. After this period the rate of moderate visual loss was 28.6% and CSME was improved in 23.8% of eyes. According to our study, baseline visual acuity and retinopathy severity were two important intervening factors in response to lasertherapy. Comparing our results with natural course of diabetic macular edema, indicates that in assessing visual outcome laser photocoagulation is an effective modality in treatment of CSME, but it is not effective in maintaining or improving visual acuity, which is due to patients delay in visiting ophthalmologists and paying not enough attention to follow-up visits.

    - Verapamil produces a sustained contraction in isolated biventer-cervicis muscle of chickens between 2-8 days old. From cumulative dose-response curves, ED50 of was calculated for this effect of verapamil. when isolated chicken biventer-cervicis muscle was electrically stimulated, verapamil had no effect on twitch contractures but increased the base line tone of the muscle. Glycerol treatment of the muscle reduced the responses to acetylcholine and KCl but had little effect on contracture produced by verapamil, and no effect on contracture produced by caffeine. Incubation of the muscles with calcium-free Krcbs solution omitted the responses of the muscle to acetylcholine and reduced the response to caffeine. Again, the responses to caffeine and verapamil were less affected compared to KCL. Addition of ethylene glycol tetra-acetic acid (EGTA) (2.5 mM) abolished the responses of muscle to all compounds. It was concluded that verapamil produces contracture of the muscle by release of calcium from intracellular stores.

    Coronary artery disease is one of the leading causes of morbidity and mortality especially in the indusirialzcd societies. This retrospective study was carried out on 385 patients who were referred to Shariati and Jamaran Hospitals from 1992 till 1997 and who underwent coronaty artery bypass grafting. The objective was to obtain a descriptive analysis of the important factors in this population and to draw a comparison betweent the two genders and to draw genders regarding these variables. The data were obtained from patients' files, angiography and operation notes; 82.9% of the study population were of male. The mean age of women was higher than men by 2.2years. The most common risk factors among the male gender were found to be smoking, hyperlipedemia, hypercholesterolemia and hypertension. Except for smoking which was omitted in women, all other risk factors showed a comparative low prevalence in this gender. Most patients (93.37%) fell in group II and III of functional class as per NYHA classification and the most frequent signs and symptoms on admission being chest pain (81.5%) and dyspnea (11.2%). The affected vessels in order of frequency were the left anterior descending, the right coronary and the circumflex arteries respectively. The average number of grafts utilized were 3 in the entire population. Overall mortality recorded was 2.1%.

    Phaeohyphomycosis is an amalgam of clinical diseases caused by a wide variety of dematiaceous fungi. We are reporting on a 16 year-old patient from Amol with subcutaneous cervical nodes and nasopharyngeal lesions of phaeohypho
    mycosis that were confirmed by pathological examination, direct smear, and culture. After treatment with an oral triazole (Itraconazole) for 4 months, all nodes and lesions disappeared and treatment was stopped A new lesion appeared on his chest wall 8 months, therapy with itraconazole was restarted and commuted for a long time.

    The goal of this prospective study was to determine die prevalence of certain surgical findings in Iranian patients with appendicitis in one of our surgical centers, including: position of the appendix, rate of true appendicitis, perforation, and peritonitis. Duplication was observed in 4%, surface exudate in 52%, gangrenous necrosis in 27%, swelling in 79%, appendix in the left lower quadrant in 2%, and mucocele in 2% of the cases.

    In this study, the effects of acute and chronic lithium on nociception induced by formalin and its interactions with morphine-induced analgesia were studied in mice. Formalin test was selected to study nociception. Subcutaneous injection of different doses of morphine (1.5, 3 and 6 mg/kg) induced analgesia in a dose dependent manner in both phases of the formalin test. Intraperitoneal administration of different doses of lithium (20, 40, 60 and 80 mg/kg) induced analgesia in late phase but didnot show any significant effect in early phase. Acute administration of lithium (40, 80 mg/kg) increased analgesia induced by morphine (3 mg/kg) in both phases of formalin test. Prctreatment of the animals with chronic lithium (1200 mg/l) for 15 days induced antinociception and increased .the analgesia induced by morphine in both phases of formalin test. It is concluded that administration of acute and chronic lithium can probably affect pain response in both phases of formalin test and interact with morphine-induced antinociception.

    The first epidemiologic survey of childhood allergic rhinitis in Iran was carried out from May 1995 until April 1996.. A questionnaire was distributed among 4584, 11-15 year-old children of both sexes, to be completed under supervision of medical students. Nasal smears were collected from the cases with allergic rhinitis (n = 445, 5.8%.), and from 340 healthy children for comparison. Significant nasal eosinophilia was present in 62% (274) of the chidren with allergic rhinitis. Classic allergic rhinitis was seen in 226 cases. In conclusion, allergic-rhinitis is one of the major health problems in Shiraz school children. Eosinophilia of nasal secretions has a specificity of 96% and sensitivity 62%, and seems to he somewhat valuable as a screening test for nasal allergy.

    Detection and modification of the risk factors of stroke may be the most effective strategy for preventing its often irreversible consequences. A longitudinal prospective study was implemented to evaluate the effect of several risk factors on the course of cerebrovascular disease. The study groups were composed of 3S8 normal volunteers, and 308 patients with transient ischemic attacks. The two groups were followed for 4.2 (. 3.3) and 2.S years, respectively. Endpoints were defined as the occurrence of the following: transient ischemic attacks, stroke, multii-infarct dementia, dementia of the Alzheimer's type, or death. .Stroke and death were 2.5 times more frequent in the second group. Hypertension was the single predictor of reaching the endpoints (P<0.014). By excluding the cases with dementia of Ahheimers type, no single predictor could be identified. This study suggests that ischemic events arc significantly more frequent inpatients with transient ischemic attacks. A dichowmous split was observed between neurogenic and cardiogenic endpoints.

    In this report we introduce two cases of penile agenesis. The first case was a newborn with male appearance and a well developed scrotum containing two normal testes, but the penis and urethral meatus were absent. The bladder was connected to the rectum by an intra-abdominal canal. The second case was a premature newbron of 32 weeks, who died in the first day of life. In addition to penile agenesis, he had undescended testes, imperforate anus, duodenal atresia, annular pancreas, kidney agensis, and toe anomalies.

    Heterophyidosis is a fish-transmitted disease in Khuzestan province, therefore a total of 900 fresh water fishes belonging to Cyprinus carpio, Liza abtt and Gambosia affinis, were collected from canals of Mazreeh district in the central area of Khuzestan. In 44%, metaccrcariae of three small flukes of human including Haplorchis taickut, Haplorchis pumilio, Stellanichasmus falcamtus were found. These metacercariae, are reported in Iran for the first time.

    Florid cutaneous papillomatosis (FCP), is a rare cutaneous marker of internal malignancy. The underlying cancer is usually intra-abdominal (most often gastric in origin), and evolves parallel to the FCP. There is a common association between FCP and the other two eruptive paraneoplastic syndromes, i.e.malignant acanthosis nigricans and the sign of Leser-Trelat. It seems possible that these syndromes develop via a common pathogenic pathway due to production of a factor similar to human epidermal growth factor by the underlying cancer. We report a case of FCP associated with malignant acanthosis nigricans, palmoplantar keratoderma and gastric adenocarcinoma.
