    This paper deals with the assessment of the depth of the modern anesthesia. A new classification is proposed and a test is designed for monitoring the movement of the hand during complete relaxation.

    Pretibial myxedema is an uncommon manifestation of Graves' disease and because of its rarity, information regarding its natural course and its relationship with other manifestations of Graves' disease is not sufficient.
    We reviewed 150 consecutive cases diagnosed as having pretibial myxedema in a twenty-year period in a tertiary care center. Only one patient in this group did not have ophthalmopathy, and the majority of cases had significant proptosis and ophthalmopathy, 30% required orbital decompression surgery. Dermopathy is a late manifestation of the Graves' disease and its onset is usually after the diagnosis of hyperthyroidism and ophthalmopathy. In a few patients, dermopathy preceded diagnosis of hyperthyroidism or the onset of ophthalmopathy. Fourteen patients have never had hyperthyroid, eleven patients in this group had developed spontaneous hypothyroidism.
    All cases showed the involvement of the lower extremities, and only one patient showed to have combined upper and lower extremities involvement. The most common form of thyroid dermopathy was non-pitting edema. Nodular and plaque forms were also relatively common and occurred with equal frequency. Polypoid form occurred in one patient and elephantiatic form in another. There was no consistent correlation among different types of dermopathy and severity of eye disease.
    During a three-month to nineteen-year follow up in 120 patients, complete remission was observed only in twelve patients. Partial remission was more common and occurred more frequently in patients who had local steroid therapy. it is possible that patients with remission might have been excluded of the follow up program.Thus, the remission data should be interpreted cautiously.

    This study was undertaken to assess the effectiveness of 5-FU plus epirubicin in the treatment of the advanced gastric cancer. For this purpose, 22 patients with no previous treatment were studied. All of the patients were unoperable or metastatic. Follow up checking continued for 24 months. Our selected therapeutic regimen consisted of a combination of 500 mg/m2 5-FU dialy for five days, and 75 mg/m2 epirubicin for one day. The response rate in this study was 47.4% and the mean survival rate was 9.7 months. By far, the most common complication was gastrointestinal disturbances and no carditoxicity was seen. Having presented the results of this study and comparing it with the other therapeutic regimens like FAM combination, this selected two drug combination seems to be a useful therapeutic protocol.

    Fourteen patients with bilateral low frequency sensorineural hearing loss were found with disproportionate speech discrimination scores. These patients were submmitted to a comprehensive audiological, electrophysiological, neurological, and radiological battery of investigation. The results pointed to a lesion probably in the central auditory pathway.

    Fifty-seven patients suffering from systemic lupus erythematosus with different clinical features were studied from various aspects of immunological abnormalities, including anti-nuclear antibodies; anti-DNA; anti-Sm (Smith's antigen); and, other extractable nuclear antigens.
    The tests employed were microhemagglutination, counter immunoelecterphoresis, and radio immunoassay (RIA).
    The comparison between the two techniques for detectiong anti-DNA titers showed a significant correlation between them. There was also a significant positive correlation between the presence of anti-DNA and lupus nephritis (P>O.05). However, lupus nephritis has a negative association with anti-Sm.
    We will discuss anti-Sm in SLE patients that may have an inhibitory effect on developing the lupus nephritis. The association of anti-Sm in SLE may help the physicians to prognose the disease and manipulate the treatment.

    In a cross section of the patients scheduled for various operations, the plausability of lidocaine as an induction agent is studied and described. The results were dismaying suggesting, henceforth, that as an induction agent, lidocaine altogether fails to accomplish the assignment assigned to this drug.

    Parasitic colitis, especially Entamoeba histolytica, is a well-known disorder in the Middle East particularly in Iran. The carrier state of the disease or the chronic type frequently remains undiagnosed or mistaken as irritable bowel syndrome which results in further discomfort and increases the incidence of associated complain. We devised a technique by inserting bisacodyl suppository and then examining the mucosal discharge for finding parasitic organisms. In our study over a period of two years on 1600 patients, which is presented here, 20% of patients who were positive underwent treatment. All signs and symptoms were disappeared after a course of treatment.

    Two hundred and forty patients, who had undergone vasectomy from 1977 to 1985 and subsequent vasovasostomy ,were studied for the presence of sperm-specific antibodies by using the Kibrick's gelatin agglutination test. The number of successful pregnancies and the presence of agglutination were also considered in this survey. Sixty-nine pregnancies occurred in total and agglutination was present in 49% out of 51% positive specimens by the Kibrick Test.
    The average sperm motility was slightly higher in the negative Kibrick group than in the positive Kibrick group. The obtained data indicated that there seems to be a relationship between the increased titers and percentage of agglutination in semen samples.

    Cavernous hemangioma is a rare benign tumor of the internal auditory canal (IAC) of which fourteen cases have been reported so far.
    Tinnitus and progressive sensorineural hearing loss (SNHL) are the chief complaints of the patients. Audiological and radiological planes, CTScan, and magnetic resonance image (MRI) studies are helpful in diagnosis. The only choice of treatment is surgery with elective transmastoid trans¬labyrinthine approach. And if tumor is very large, the method of choice will be retrosigmoid approach.

    The close relationship between the immune system and the central nervous system (CNS) has been well-studied. The CNS regulates almost all the functions of the immune system; however, the effect and/or the control of the immune system on the CNS has not been well-studied yet. Recent reports suggest that the CNS has its own defence mechanism(s). In this review, attempts have been made to discuss some of the controversial issues in order to approach a clear conclusion.

    In a comparative study, 460 serum samples were tested for the serological determination of human immunodeficiency vrius (HIV) using an ELISA kit and a Rapid Test kit. Results indicated that the Rapid Test is reliable for screening unkonwn samples when there is no special equipment for screening with immunoblotting and/or ELISA.

    Verrucous carcinoma is a unique clinical and pathological entity which has been recognised in the recent years. It is a variant of well-differentiated squamous cell carcinoma which may occur in the larynx; other anatomic locations such as: the oral cavity, anogenital region, and skin may be involved. It accounts for 1-2% of all laryngeal cancers. The clinical, histopathological and biologic features of this neoplasm differ from that of conventional squamous cell carcinoma. Therefore, close communication between pathologist and laryngologist is necessary for a correct diagnosis.
    Eleven cases of verrucous carcinoma which were collected in our institu¬tion within the past ten years are going to be reported. The clinical and pathological features will be discussed. It seems surgical excision is the treatment of choice and irradiation must be discouraged since anaplastic transformation and recurrences after irradiation has been reported.

    These investigations indicate that the anterior layer of the renal fascia continues across the midline of the body in front of the aorta and inferior vena cava to become continuous with a similar layer on the opposite side of the body. The posterior layer of the renal fascia blends with the fascia covering the psoas major especially at the medial and lateral edges of this muscle. But, in addition to these two layers, there is also a deeper stratum connecting the anterior and posterior layers around the medial border of the kidney, and this deeper stratum is pierced by the renal vessels and ureter. At the upper pole and lateral border of the kidney, the anterior and posterior layers of the fascia fuse; at the lower pole they also fuse, but much more loosely than in the other locations. Incidentally, the perirenal veins maybe sufficiently large to merit attention during operations on the kidney.
